Social Prescribing

Introducing Karen Price – Social Prescriber:

Hi, my name if Karen Price and I am your local Social Prescriber, based in the North Cotswolds. I work for The Community Wellbeing Service which is hosted by Gloucestershire Rural Communities Council. This is a free and confidential service for people over the age of 16.

In the North Cotswolds we consist of a team of 3; a Social Prescriber and 2 Community Wellbeing Agents. The Social Prescribing role is predominantly GP Surgery based and clients can book appointments to meet with me. Social Prescribing represents a means of enabling GPs and other frontline healthcare professionals to refer patients to our service to provide them with a face to face conversation (either in the Surgery or at home) for issues that are non-medical. During this conversation they can learn more about the possibilities open to them and design their own personalised solutions. This helps people with social, emotional or practical needs to find solutions which will improve their health and wellbeing, often using services provided by the voluntary and community sectors.

Advice on non-medical issues may include:

  • General health and wellbeing
  • Volunteering in the local area
  • Accessible transport
  • Age related advice and activities
  • Support with healthy lifestyles
  • Building networks
  • Getting back in to work
  • Support for long term health conditions
  • Emotional wellbeing
  • Activities within the area
  • Social isolation

  • Employment
  • Education
